Your first visit to the office establishes a vital foundation for our relationship with you. We’ll ask you for current contact and insurance information and go over a few questions about your medical history. Please arrive 15 minutes early for your appointment.

Insurance
We accept a variety of insurance providers. Prior to your first appointment, please contact your insurance provider to confirm we are an in-network provider.
At your first appointment, please bring your insurance information or policy holder’s card, and/or group number to expedite insurance processing.
Financial Policy
For all treatments, we ask for payment of the portion of fees not covered by insurance at the time of your procedure. If you do not have dental insurance, please ask about our Healthy Smiles Discount Program.

Cancellations
We know there are times and situations when you cannot keep your appointment. Please Contact Us as soon as possible if you need to change your appointment.
